REG 343

Application for Title or Registration/Verification of Vehicle (REG 343)

This application form is used for vehicles being registered for the first time and for nonresident vehicles brought into California. It must be submitted with the appropriate fees within 20 days of the date the vehicle is first operated in California. This form may also be used to reregister a vehicle after it has been junked, salvaged, or removed from the department’s database. The REG 343 is generally submitted with other supporting documents and is not to be mailed by itself.

Notice of Transfer and Release of Liability (REG 138)

When the owner of a California registered vehicle sells or transfers title or interest in the vehicle, the seller must complete a Notice of Transfer and Release of Liability (REG 138) and submit it to the department within five calendar days. Keep a copy of the REG 138 for your own records.

REG 352

Year of Manufacture (YOM) License Plate Application (REG 352)

This form is used to apply for assignment, reassignment, or retention of a Year of Manufacture (YOM) plate. YOM plates are any officially manufactured California license plates issued prior to 1969 for auto, motorcycle, or trailers and extends to 1972 or prior for commercial vehicles that the department determines are legible and serviceable pursuant to California Vehicle Code §5004.1.

REG 4017

Permanent Trailer Identification Certification (REG 4017)

The REG 4017 is a two-sided form used for trailers in the PTI program. Side A is completed and signed by sole owners requesting a paper certificate of title. Side B is an application for: the transfer of ownership for a PTI trailer with an electronic title record or addition of a legal owner only. The transfer may be a registered owner transfer or a release of security interest only.
